The clerk of court’s office qualifies candidates for local elections.  Qualifying for regional or statewide offices is handled by the Secretary of State.  The procedure for qualifying is to produce a Notice of Candidacy (Qualifying Form) during the qualifying period with the appropriate filing fees for the office sought. You may download the form from the Louisiana Secretary of State:  http://www.sos.la.gov/ElectionsAndVoting/PublishedDocuments/QualifyingForm.pdf.  This is the official qualifying form approved by the Attorney General and provided by the Secretary of State and it must be filed with the appropriate qualifying official.

Agent Affidavit Forms: When an agent files a notice of candidacy on behalf of a candidate, the candidate shall file with the qualifying official Agent Affidavit form attesting that the agent has the authorization and consent of the candidate to file the notice.

The schedule of qualifying fees may be accessed here:  http://www.sos.la.gov/ElectionsAndVoting/PublishedDocuments/CandidateQualifyingFees.pdf.

When an individual qualifies as a candidate, he/she is provided with information prepared by the Louisiana Department of Ethics regarding campaign finance and personal finance disclosure requirements.
